Who we are

Ira S. Hofer, MD is a practicing anesthesiologist and clinical informaticist at The Icahn School of Medicine at Mount Sinai and the founder and President of Extrico Health. Dr. Hofer has been working in big data analytics with electronic health record (EHR) data for over 15 years. He has held a variety of roles at multiple world-renowned hospital systems building and deploying AI (Artificial Intelligence) models and overseeing analytics for system wide and departmental programs. He currently holds joint appointments in Anesthesiology, Medicine at the Charles Bronfman Institute for Personalized Medicine and is the inaugural director of the Rapid Randomized Trial Unit in the CBIM which is focused on deploying AI models into clinical care using study designs to measure efficacy.

Dr. Hofer’s research interests involve leveraging the data from the EHR to understand, quantify and mitigate risk in the perioperative period. His work includes some of the first papers to apply machine learning techniques for perioperative outcome prediction and has been featured on the cover of both Anesthesiology and Anesthesia & Analgesia. Ira has a K01 from the National Heart Lung and Blood institute, and funding from the NIA funded IMPACT Collaboratory for a pragmatic trial to reduce postoperative delirium using an AI driven decision support model. He also serves as an associate editor for Anesthesia & Analgesia. His current research focus is on creating featurization techniques to incorporate a wide range of EHR data into machine learning models to improve discrimination and calibration and establishing multi-center perioperative collaboratives to build more generalizable models and enhance health equity.

John LaValle comes to Extrico as the Chief Executive Officer with decades of early-stage and public company experience. This includes operating and management experience in several venture capital-funded technology startups and middle market companies. John has served multiple stints in the C-Suite, including several times as CEO, with deep experience taking companies from their earliest stages through exit. He has taken four companies through an IPO, reverse merged an entity in to a $300 million SPAC, and led one of the largest secondary offerings ever done on NASDAQ. He has raised over $2.5 billion in capital for private and public companies and has negotiated hundreds of millions of dollars in new business. John’s addition to Extrico brings the strategic planning, fundraising, and team building experience necessary in scaling the company. John graduated Summa Cum Laude from Boston College and went on to earn his MBA from Harvard Business School where he was awarded multiple fellowships.

Kevin joins Extrico from Premier, Inc. where he led the commercial operations of a $250M business unit including sales, marketing, product development, and sourcing.

Prior to Premier Inc., Kevin held executive leadership positions in several early-stage healthcare IT companies where he designed strategic business development plans and built high-performing sales, marketing and product development teams. His early experience of increasing responsibility with large medical device companies such as Stryker, Baxter and ev3 give Kevin a unique and balanced view of healthcare and in particular, the clinical space and its need for easy to use, cost-effective IT solutions.

Maria Moore is a uniquely qualified, high-integrity executive (Yale MPH), focused on serving both large and midsized hospitals and corporations. A proven, results-driven catalyst with an innovative yet independent style and a distinguished record of delivering exceptional client service. Maria has over twenty-five years of experience in healthcare leadership positions in technology start-ups (strategy, operations, hr, investor relations, finance/funding, customer relationship management), health care administration, clinical benchmarking, healthcare technology (emr/ehr, biomedical, administrative, educational, research), vendor management, enterprise-wide implementations, and program/project management driving issue resolution by fostering collaborative executive relationships and building strong teams.

Shirel Daniel is driven by an unwavering passion for storytelling and a steadfast dedication to improving health outcomes. She brings extensive experience in establishing strategic partnerships, driving business development, and spearheading marketing initiatives to expand market reach in clinical care, innovation, and education. Holding a Bachelor of Science from Syracuse University and a Master of Public Health from Tel Aviv University, Shirel has a comprehensive background spanning the private, public, and academic sectors of healthcare. Her roles at Philadelphia International Medicine, the United Nations Environment Program, and Ben Gurion University of the Negev have provided her with a deep understanding of the industry and a unique global perspective.

Sivaranjani Jayarajan is a Software Engineer with over five years of experience in backend development, API design, and system optimization across healthcare and fintech industries. She has built scalable microservices, automated large-scale invoicing systems, and developed internal tools that improve operational efficiency and data integrity. Sivaranjani is passionate about using technology to drive impact, especially in mission-driven sectors. Outside of work, she enjoys hiking, exploring new places, and discovering new cuisines.

Jennifer Miller brings over twenty years of experience in data management and software development to Extrico. Experienced in a variety of roles at environmental and healthcare companies, she believes that carefully managed and accurately reported data is essential to the long-term success of any business.  Jennifer is passionate about keeping the lines of communication open between clients, customers, and businesses to deliver exceptional solutions with outstanding customer service.

In recent years, Jennifer has been drawn to the impact that meticulously managed and reported data has on the decisions made by the industries that determine our health and environmental outcomes. Jennifer is committed to supporting the healthcare leaders and scientists who carefully collect, research, and analyze data for the future benefit of humankind.

Jennifer holds a Bachelor of Arts from the University of Georgia and resides in Atlanta.

Vijay Vempati is a seasoned Senior SQL Software Engineer with a Master's degree in Data Science and over three years of professional experience in extracting valuable insights from data.

Passionate about unraveling the stories hidden within data, Vijay excels in SQL development, database management, and data analysis.

Throughout his career, Vijay has demonstrated a strong aptitude for transforming complex datasets into actionable insights, driving informed decision-making and strategic initiatives. With a current focus on healthcare, he specializes in leveraging data to improve patient outcomes, optimize healthcare operations, and drive predictive analytics initiatives. Beyond his professional endeavors, Vijay is a multifaceted individual with a keen interest in photography and sports.

When he's not diving into data, you can find him capturing the beauty of the world through his camera lens or engaging in his favorite sports activities.

Kari McLean, MSN, RN, NI-BC, is an accomplished healthcare consultant and clinical informatics specialist with a strong focus on leveraging data to improve patient outcomes and streamline healthcare efficiency. She has extensive experience across diverse healthcare settings, from large integrated delivery networks (IDNs) to critical access hospitals (CAH), where she has supported and optimized Electronic Health Record (EHR) implementations, using interdisciplinary collaboration to drive measurable improvements in care quality and operational effectiveness.

Kari combines her clinical expertise with strong skills in data analysis, change management, and value realization. She has led major EHR projects, creating data-driven insights and visuals to support executive decision-making and developing strategies to help clients showcase their innovations and successes.

With over 20 years of commitment to knowledge-sharing and community service, Kari taught "Intro to Healthcare Informatics" at the University of Montana, Missoula College, and served as a lieutenant and advanced emergency medical technician with a volunteer fire department. Kari holds a Master’s in Nursing Informatics from Walden University and a Bachelor’s in Nursing from Montana State University. She is board-certified in Nursing Informatics (ANCC) and is a Certified Revenue Cycle Representative (CRCR) through HFMA.
